Abstract
The ultimate aim of controlled ovarian stimulation (COS) is to facilitate maximal success with minimal side effects. Multiple pregnancies with resultant maternal, fetal, and neonatal complications are an avoidable adverse outcome following IVF. This iatrogenic complication could be avoided by adopting single embryo transfer. Fine-tuning COS can lead to optimal response in terms of oocyte quantity and quality which in turn influence the subsequent events of IVF, leading to good quality embryos thus enabling selection of the single best embryo for transfer. The various events of COS including pituitary suppression, ovarian stimulation, and ovulation triggering are discussed in this chapter. The alternative approaches to each of the events are described and the concept of individualisation of COS for IVF is discussed, detailing ideal regimens for women categorised as normal responders, poor responders, or hyper-responders.
