Abstract

Bariatric surgery is now recognized as the only effective treatment for severe obese (SO) patients with long-term sustained weight-loss and post-operative complete resolution or significant improvement in the obesity co-morbidities.

Since obesity is a complex condition where biological, psychological, and social factors interfere to lead to excess body weight, its management cannot focus only on weight reduction. Selection of the eligible patient for surgical intervention, interdisciplinary preoperative preparation, amelioration of obesity co-morbidities, particularly type 2 diabetes, long-term sustained weight-loss with maintenance of fat-free mass, improving quality of life, dietary, and exercise advises are included in treatment terms.

Therefore, comprehensive obesity management by either conservative or surgical method is “by definition” a multidisciplinary effort and should be undertaken by a team of different professionals able to tackle with the different aspects of obesity and its related disorders.

In Europe, the establishment of a Center of Excellence (CoE) program under the guidance of the European Chapter of IFSO (IFSO-EC) and the administration of the European Accreditation Council for Bariatric Surgery (EAC-BS) has proved that safe and effective surgical management of morbidly obese patients is offered in institutions with the necessary multidisciplinary experts and multilevel resources.
