Off-Label Use of Aortic Devices and Parallel Stent Grafts for Iliac Revascularization

Endovascular Aortic Repair

Abstract

One-third of the patients treated for abdominal aortic aneurysms (AAAs) with endovascular aortic aneurysm repair (EVAR) have ectatic or aneurysmal common iliac arteries. Exclusion of the internal iliac arteries has been widely utilized, but carries a definite risk of buttock claudication or ischemic pelvic complications. The off-label use of approved stent grafts to preserve internal iliac artery flow has originated from the lack of widespread access to specifically designed iliac branch devices (IBDs). This chapter summarizes basic concepts and techniques of iliac parallel stent grafts.
