Abstract

Migraine is a widely prevalent, complex neurobiological disorder with headache as its primary manifestation, but also including a variety of neurologic symptoms and phenomena. Migraine affects all ages, geographies, and races and creates significant personal disability as well as personal and societal costs. Although past theories implied a primarily vascular nature to the headaches, recent understanding of the pathology of migraine includes recognition of cortical and subcortical activation, genetic predisposition, cranial nerve sensitization, and neurovascular inflammation. While migraine without aura is the most common form, a variety of other migraine conditions are recognized. Treatment options for these conditions are expanding, including pharmacological and behavioral therapies, both to prevent the attacks and to abort them.
