Abstract

Post-operative recurrence of Crohn’s disease is a significant problem with endoscopic recurrence rates reported to be between 70 and 90 % within 1 year. In addition 70 % of patients will proceed to a second operation over the course of their lifetime. Prevention of this post-operative recurrence is essential and the post-operative setting should be viewed as the ideal opportunity to prevent symptomatic disease and need for future surgery. Utilizing a preventative and treatment strategy based on patient risk with frequent objective monitoring of disease and titration of medical therapy based on these objective findings, future disability, and surgery may be avoided in the majority of patients.
