Abstract
The incidence of periprosthetic fractures involving TKR is increasing. There is a significant morbidity associated with these fractures, and they present a substantial technical challenge for the orthopaedic surgeon. These are complex injuries best treated by surgeons experienced in both trauma and revision arthroplasty. Femoral fracture treatment is guided by both the patients’ comorbid status and the fracture configuration. Tibial fractures are less common. Open reduction and internal fixation and/or revision to a stemmed tibial component are often necessary to gain adequate fixation if the primary prosthesis has become loose. Patellar fractures continue to have a poor prognosis functionally for the patient in all but the minimally displaced injuries with preservation of the extensor mechanism. This chapter reviews the epidemiology and fracture classifications of tibial, femoral and patellar periprosthetic fractures after TKR.
