Quantitative and Qualitative Immune Reconstitution Following Umbilical Cord Blood Transplantation

Abstract

Delayed neutrophil and platelet engraftment as well as slow reconstitution of T, B, natural killer, and dendritic cells have been observed after umbilical cord blood transplantation (UCBT). Even after quantitative recovery, there are qualitative differences in immune reconstitution after UCBT compared to recovery after transplantation of an adult stem cell source. The cord blood graft has a unique composition and pattern of cellular programming. Several lines of evidence indicate that lymphocyte reconstitution after UCBT is characterized by restricted T cell repertoire diversity. This results in a low incidence of graft-versus-host disease but an accompanying high rate of infections. Detailed studies of immune reconstitution are being carried out to enable new approaches to enhance immune function and to prevent infectious complications and transplant-related mortality after UCBT.
