Indications to Laparoscopic Cholecystectomy

Abstract

The panel of 1994 EAES Consensus Conference on LC answering to the question “Who should undergo an LC?” stated that LC is indicated in symptomatic gallbladder disease patients who are able to tolerate general anesthesia including patients with porcelain gallbladder. On the other hand the panelist identified two special subgroups of patients:

  • Symptomless gallstone cases that should be followed up closely (!) (diabetic, sickle cell anemia, those on long-term somatostatin treatment, etc.)

  • Patients who required “extreme caution” and an “expert surgeon” (acute cholecystitis, pregnancy, elderly, etc.)

Finally the absolute contraindications were reported based on the 1994 evidence. During the last 18 years LC has been accepted in the surgical and medical community and is the most diffuse laparoscopic procedure worldwide. A large number of papers have been published during this long interval of time and the indications for LC have to be revisited according to the new evidence provided by the literature. The panel selected among the several “hot topics” related to LC indications the following 7 ones. The panel agrees that the contraindications stated in 1994 remain unchanged and still valid and excluded children from the systematic review of the literature.
