Zusammenfassung
Das Biofeedback des Körperschwankens stellt eine Möglichkeit dar, Patienten mit Gleichgewichtsproblemen alternative Sinnesreize zur Verfügung zu stellen, was zu einer Verbesserung ihrer Lebensqualität führt. Als Ansatz wählten wir ein multi-modales Biofeedback-Vorgehen, das den Patienten entscheidende Sinnesinformationen über das Schwanken ihres Körpers liefert. Wir gingen von der Annahme aus, dass über am Kopf befestigte Signalwandler auditive, vibro-taktile und vibro-vestibuläre sinneseindrücke erzeugt werden können, die dazu führen, dass wenig oder gar nicht genutzte sensorineurale Informationen zum Gleichgewichtszentrum aktiviert werden. Für diesen Zweck werden die künstlich erzeugten Biofeedback-Signale mit einer globalen Messung des Körperschwankens kodiert; diese Messung geschah mit Hilfe von Neigungswinkelgeschwindigkeits-Sensoren, die in der Nähe des Körperschwerpunkts angebracht wurden. Die Ergebnisse sowohl an jungen als auch an älteren Probanden ergaben, dass mit Hilfe des Biofeedbacks eine bemerkenswerte 40–60%ige Reduktion des Körperschwankens erzielt werden konnte, die deutlich grösser ausfiel als mit einem reinen Gleichgewichtstraining. Zukünftige Untersuchungen sollen die Frage beantworten, ob ein langfristiger Lerneffekt der Biofeedback-Anwendung existiert. In jedem Fall haben wir die Rahmenbedingungen für die Entwicklung einer Prothese geschaffen, mit deren Hilfe Gleichgewichtsprobleme reduziert werden können. In Zukunft kann ein derartiges System auf die Größe etwa eines Hörgerätes miniaturisiert werden.
