Zusammenfassung
Der Begriff Osteomyelitis bezeichnet die Infektion des Knochens; er wird meist sowohl für den Befall der Corticalis als auch des Markraumes benützt. Bei einer Infektverschleppung von außen (per continuitatem) bei chronischen Wunden (Dekubitus, diabetisches Fußulcus, arterielles oder venöses Ulcus) kommt es zuerst zu einer Beteiligung des Knochencortex (Osteitis) und erst bei Fortschreiten zu einer Beteiligung des Markraumes (Osteomyelitis). Eine Infektion des Knochens kann aber auch hämatogen entstehen, oder viel häufiger posttraumatisch nach offenen, seltener geschlossenen Frakturen mit oder ohne durchgeführte Osteosynthese. Die Entwicklung der modernen Endoprothetik hat als schwerwiegende Komplikation ebenfalls zur Zunahme der Knocheninfektion beigetragen, wobei Biomaterialien und deren hohe Affinität zu Bakterien als besonderer Aspekt zu berücksichtigen sind. Aus diesem Grund sind es Ärzte verschiedenster Fachdisziplinen, die sich mit der Diagnostik und Behandlung der Knocheninfektionen beschäftigen. Pädiater und Internisten behandeln hämatogene Formen, Traumatologen posttraumatischen Formen, Orthopäden solche nach Gelenksersatz, Gefäßchirurgen und Angiologen jene durch arterielle Problemwunden. Allgemeinmediziner und Diabetologen, die das diabetische Fußsyndrom betreuen, sind mit einer Knochenbeteiligung am Fuß konfrontiert. Plastische Chirurgen werden häufig erst zur Defektdeckung nach radikalem Debridement hinzugezogen. Durch die Einführung der systemischen Antibiose haben die Knocheninfektionen zwar an Schrecken verloren, jedoch gilt es, eine Chronifizierung des Infektes mit Sequestrierung und rezidivierender Fistelung unbedingt zu verhindern. Beim diabetischen Fußsyndrom ist die Knochenbeteiligung oft der limitierende Faktor für einen Extremitätenerhalt.
Die Diagnostik bei Osteomyelitis erfolgt klinisch, mikrobiologisch und histologisch. Als bildgebende Verfahren kommen das konventionelle Röntgen, die Skelettszintigraphie, die Computertomographie und die MRT-Untersuchung zum Einsatz. Die Behandlungsprinzipien der chronischen Osteomyelitis sind: (1) Radikales chirurgisches Debridement (Knochen und Weichteile) (2) Keimgewinnung und Isolierung sowie kulturgerechte systemische Antibiose (3) Weichteilrekonstruktion und Totraummanagement durch gut vaskularisierte gestielte oder freie Lappenplastiken (4) ev. Knochenrekonstruktion (Ilizarovtechnik oder vaskularisierter Knochentransfer).
