Zusammenfassung
In der Behandlung von venösen Unterschenkelgeschwüren ist die Kompressionstherapie die wichtigste Therapiekomponente. Sie reduziert Ödem und Schmerzen, verbessert die venöse Beinpumpe und bessert die Mikrozirkulation. Entscheidend sind ausreichend hohe Drücke, die vor allem mit guten Kompressionsverbänden, neuerdings zum Teil auch mit geeigneten Kompressionsstrümpfen erzeugt werden können. Zur lokalen Drucksteigerung über dem Ulkus eignen sich Schaumgummi-Polster. Wirksame Kompressionsverbände bedürfen einer Einschulung. Vor Anlegen von festen Verbänden und Strümpfen muss die arterielle Durchblutung kontrolliert werden, am einfachsten in Form einer Doppler-Ultraschall-Untersuchung. Die zusätzliche Verwendung von intermittierenden pneumatischen Kompressionspumpen ist vor allem bei wenig oder nicht mobilen Patienten sowie bei Patienten mit Beinödemen und begleitenden arteriellen Verschlüssen als hoch wirksam zu empfehlen. Unter einer sachgemäß durchgeführten Kompressionstherapie sollte die Heilrate von konsekutiv behandelten, rein venösen Beingeschwüren innerhalb von 12 Wochen bei 70 % liegen.
