Bei den epithelialen, d. h. von den Follikelzellen abgeleiteten Schilddrüsenkarzinomen unterscheidet man die drei Kategorien der papillären (PTC), follikulären (FTC) und anaplastischen (ATC) Karzinome (Abb. 8.1). Die papillären sowie die follikulären Karzinome, einschließlich des oxyphilen (Hürthle-Zell-)Typs, werden als differenzierte Schilddrüsenkarzinome (DTC) zusammengefasst. Obwohl die UICC/AJCC in ihrer TNMKlassifikation (Sobin und Wittekind, 2002) und die Mehrzahl der Studien PTC und FTC als eine Entität mit identischer chirurgischer Therapie betrachten, gibt es Unterschiede zwischen diesen Karzinomen in endemischen Strumaregionen: PTC-Patienten haben dort eine signifikant bessere Prognose als solche mit einem FTC (Passler et al., 2004). Das maligne Potenzial der Schilddrüsenkarzinome zeigt eine große Variabilität und reicht vom meist „benignen“ Verhalten papillärer Mikrokarzinome bis hin zum nahezu immer letalen Verlauf bei undifferenzierten oder anaplastischen Karzinomen.
