Zusammenfassung
Die Acetylsalicylsäure-sensitive Erythromelalgie ist kausal mit einer Thrombozythämie verbunden und wird durch Blutplättchen-vermittelte fibromuskuläre Proliferation und thrombotische Verschlüsse in der endarteriellen Strombahn der Extremitäten in Abwesenheit einer vorbestehenden Gefäßkrankheit verursacht. Klinische Symptome der Thrombozythämie sind nicht nur die Erythromelalgie und ihre peripheren ischämischen Komplikationen, sondern auch eine hohe Inzidenz von Blutplättchen-vermittelten mikrovaskulären zerebralen Symptomen und Herzgefäßkrankheit. Eine niedrige Dosis von Acetylsalicylsäure heilt und verhütet die Erythromelalgie und die ischämischen Mikrozirkulationsstörungen des Gehirn und Herzens durch irreversible Hemmung der Cyclooxygenase-Aktivität in den Blutplättchen. Klinische, experimentelle und plättchen-kinetische Studien bestätigen die Existenz einer Plättchen-vermittelten Genese der Erythromelalgie und aller anderen ischämischen Mikrozirkulationsstörungen bei Patienten mit essentieller Thrombozythämie und Thrombozythämie bei Polycythaemia vera.
Summary
Acetylsalicylic acid-responsive erythromelalgia is causally related to thrombocythaemia and caused by platelet-mediated fibromuscular intimai proliferation and thrombotic occlusions in the endarterial circulation of the extremities in the absence of a pre-existent vascular disease. Erythromelalgia and its peripheral ischaemic complications as well as high incidence of platelet-mediated typical and atypical transient cerebral ischaemic attacks and coronary artery disease are the main presenting symptoms of thrombocythaemia. Low-dose acetylsalicylic acid cures and prevents erythromelalgia and the ischaemic microcirculatory disturbances of the cerebral and coronary and peripheral circulation by irreversible inhibition of platelet cyclooxygenase activity. Clinical, experimental and platelet kinetic studies confirm the existence of a platelet-mediated genesis of erythromelalgia and cerebral and coronary ischaemic symptoms in ptients with essential thrombocythaemia and thrombocythaemia associated with polycythaemia vera.
