Abstract

The leishmaniases form a constellation of diseases resulting from invasion of the reticuloendothelial (or, lymphoid-macrophage) system of a vertebrate host by hemoflagellates of the genus Leishmania. Ordinarily, the parasites are transmitted by the bite of a small, hairy sandfly of the family Phlebotomidae, in which there is a developmental cycle in the mid-gut. Multiplication of parasites at intracellular sites ordinarily begins around the site of the original infection. There may be limitation of the organisms to that area, or may metastasize to other locations in the lymphoid-macrophage system. Clinical manifestations of diverse character result from such infections, as indicated by the terms cutaneous, muco-cutaneous, and visceral leishmaniasis. The taxonomy of the Leishmania has posed numerous and complex problems, and strains or demes of the organisms may alter response of the patient to both the parasites and to agents used in treatment of the leishmaniases.
