Abstract

This chapter explains the surgery of a difficult diabetic vitrectomy which was never treated with laser. These patients have a poor compliance and present at the ophthalmological department with severe visual acuity decrease and advanced diabetic retinopathy. Many VR surgeons perform an immediate vitrectomy. We recommend instead an immediate treatment with anti-VEGF and laserphotocoagulation and a delayed vitrectomy. The technique of laser indirect ophthalmoscopy is described in detail.
