Abstract
The paediatric surgeon provides a component of the continuum of care to the paediatric patient. The process is initiated once a prenatal diagnosis is established which will require the surgeon’s expertise in conjunction with other specialties, in counselling the family and establishing a management plan. Once the decision to perform surgery has been made, a careful pre-operative assessment is essential and requires understanding of the unique physiology and pathophysiology of paediatric and especially neonatal patients. This includes clinical examination, cardiopulmonary evaluation, body temperature control, blood work-up and rigorous resuscitation. A reliable form of vascular access may be required and securing one can be sometimes challenging in a paediatric population, especially in neonates and infants. This should be planned carefully with selection of the appropriate insertion technique and vascular access device, based on a number of criteria, such as age, infusate characteristics, therapy duration and frequency. All these aspects are discussed in detail in this chapter.
