Abstract
This chapter explains the structural requirements for fertility counselling. Special focus is placed on fertility counselling for children and adolescents, in whom fertility preservation is becoming increasingly important. The background to this is the significantly improved chances of survival after oncological disease in childhood and adolescence and the fact that health insurance companies in an increasing number of countries are covering the costs of fertility-preservation measures. Successful fertility counselling for both children and adults requires a multidisciplinary team with an internal consensus based on current guidelines for fertility preservation, established procedures for providing fertility preservation measures to patients, structural and legal requirements for the removal, processing and storage of tissue and documentation of the counselling and measures taken. It is important that the various factors influencing the indication and use of fertility preservation measures (underlying disease, metastasis, stage of puberty, general state of health, available time, prognosis) are appropriately assessed in order to be able to provide patients with individual advice.
