Abstract
Peroneal tendinopathy is a common source of lateral-sided foot and ankle pain. Once the tendons and/or muscle bellies have become severely degenerated or rupture, repair may not be an option. Additionally, allograft reconstruction is contraindicated if the peroneal muscle belly is nonfunctional as in cases of atrophy or significant scarring. Flexor digitorum longus (FDL) transfer to the base of the fifth metatarsal is a viable option for nonrepairable/nonreconstruction peroneal tendons. The FDL transfer is a safe and successful procedure which restores function and reliably decreases pain.
