Abstract
The prepuce consists of three parts body of the prepuce, median raphe and the fraenulum. Shape and abnormalities gives clue about the congenital anomalies and their severity of penile and urethral anomalies. The hooded Foreskin/prepuce or dorsal hood when the foreskin of a boy is wide open and is indicative of hypospadias. Similarly the ending of median raphe on the prepuce and its deviation from midline co-relates with the degree of torque. The site of bifurcation of median raphe and site dorsal hump corroborated with the severity of hypospadias. So careful examination and evaluation of prepuce is helpful in planning the treatment of the hypospadias. There are various classification of hooded prepuce and these are useful in planning the preputioplasty. It is necessary to measure the adequacy of prepuce by various methods given herein before going for preputioplasty. Three layer preputioplasty is an easy technique and creating aesthetically a normal looking penis by adding 15–20 min extra in hypospadias repair. Minor complication like prepucial oedema phimosis and dehiscence can be prevented by choosing the proper case for preputioplasty without increasing the complication of urethroplasty.
