Abstract

Treatment nonadherence, the extent to which a person’s behavior matches medical advice, is critical for the maintenance of optimal health outcomes and is a major public health concern; half of pediatric patients are nonadherent to their prescribed medications. A hospital admission or outpatient medical clinic visit provides the CL psychologist with the opportunity to identify difficulties with adherence, reinforce the importance of treatment adherence, and implement tailored strategies for improving adherence. This chapter examines the use of the pediatric self-management model (Modi, A. C., Pai, A. L., Hommel, K. A., Hood, K. K., Cortina, S., Hilliard, M. E., … Drotar, D., Pediatrics 129:e473–485, 2012) as a guide for (1) conceptualizing adherence within a contextual framework, (2) assessing treatment adherence and related barriers, (3) providing evidence-based adherence interventions and recommendations within the context of CL service, and (4) communicating adherence-related issues with the physicians, nurses, and other health-care providers. Specifically, components of a comprehensive adherence assessment, including medical chart review, interviews with the patient and family, and input from the medical team as well as effective adherence interventions utilizing educational, behavioral, organizational, and combined approaches are discussed.
