Abstract
The incidence of pelvic fracture urethral injuries is increasing the world over. Simple cases can be managed locally. However, there are cases which are complex. The management of such cases is challenging and should be done by experts. Recently there have been publications highlighting complex posterior urethroplasty. Our center is a tertiary referral center and we have had the highest number of complex posterior urethroplasty cases published in the literature to date. The referrals are increasing each day. We have categorized complex pelvic fracture urethral injuries into eight scenarios, which we have seen over the last two decades. These scenarios include re- and re-redo cases with multiple prior urethroplasty, bulbar necrosis, boys <12 years, girls with pelvic fracture urethral injuries, double block at bulbomembranous junction and bladder neck prostate, bladder neck incontinence, rectourethral fistula and concomitant anterior and posterior urethral strictures. This article highlights each category of complex pelvic fracture urethral injury.
