The Reserve of Hip Extension and Its Relationship with the Spine

Abstract

The reserve of hip extension can be defined as the amplitude of maximum extension of the coxo-femoral articulation relative to the vertical neutral position.

If there is a limitation during displacement, the femur with every step would force the pelvis to repeated extension and the spine to repeated hyperlordotic movements, leading to degeneration and back pain.

It can be evaluated by clinical examination, by optoelectronic methods, and as we will outline, by radiological measurements using specific images.

The extension reserve appears as a single factor that is not closely related to the parameters of sagittal balance.

The rehabilitation with relaxation of sub-pelvic segments appears useful, but it cannot compensate beyond a certain limit.

For surgery, prevention is mandatory. In case of sagittal balance correction, this parameter should be taken into consideration.
