Abstract
Restrictive lung diseases (RLDs) are disease processes which limit expansion of the respiratory system resulting in a reduction in lung volumes when measured by pulmonary function tests (PFTs). Intrinsic lung diseases result when there is an increase in lung elastic recoil due to scarring or infiltrative processes. These physiologic abnormalities can lead to several increased perioperative risks. Currently, there are no evidence-based guidelines for the perioperative management of RLD. Preoperative evaluation should include a detailed history of functional status and other risk factors for postoperative pulmonary complications. Postoperative recommendations are based on the underlying etiology of disease and a pulmonary consult is warranted in patients with severe respiratory compromise.
