Abstract
The transformation of healthcare envisioned by the Institute of Medicine in their “Quality Chasm” report has led to a great many changes in primary care, but the change required to make patients full partners in their care has yet to be achieved in most settings. There is a population of patients with complex health needs and trauma histories for whom this partnership is a requirement if they are to be able to develop the self-efficacy required to be effective at self-management of their health challenges. Using methods that are becoming more widely implemented such as OpenNotes, Solution-Focused Interviewing, Patient Activation language, and Patient-Centered Care Plans, a coherent model for addressing the needs of these patients can be described. It is Transparent, Empowering, Activation and Mutual: the T.E.A.M. Way.
