Primary Surgery Versus Organ Preservation in Advanced Laryngeal Cancer

Abstract

Over 4500 North Americans will be affected by advanced laryngeal cancer in 2018. As cancer treatments have progressed, treatment paradigms have shifted from “survival at all costs” to “survival with maximum functional and quality of life outcomes.” Treatment options range from organ preservation with chemo-radiation therapy (CRT) to total laryngectomy (TL) with adjuvant RT. The question is: which treatment options offers the optimal balance between cure, function and quality of life?

This systematic review examines the overall survival as well as other survival outcomes and functional outcomes in patients with T3 or T4a laryngeal cancers treated with primary CRT vs primary TL. All relevant papers were examined and data was synthesized to propose a modern approach to patients with advanced laryngeal cancers.
