Abstract
The majority of patients presenting with acute diverticulitis can be managed successfully with medical therapy. In the stable patient, a careful preoperative evaluation and discussion of treatment options can guide procedural or elective surgical interventions as needed. Disease progression resulting in an unstable clinical picture, diffuse peritonitis, large bowel obstruction, or failure of medical management should prompt expeditious surgical intervention. A comprehensive understanding of surgical indications and options in the management of acute diverticulitis is required to treat what may be a potentially lethal event for these patients.
