Abstract
This chapter describes the development and organization of psychiatric care in Croatia, starting from the nineteenth century when psychiatric patients were considered dangerous and placed in asylums up until today when psychiatric treatment tends to be community-based. A special emphasis is put on forensic psychiatry and the changes it has gone through not only due to advances in medicine and, specifically, in psychiatry but also due to changes in law. There are three sectors involved in the treatment of mentally disordered offenders and “long-stay” forensic patients in Croatia. These include four forensic departments in psychiatric hospitals, which are low to medium security, the prison system with prison hospitals and prison psychiatric services for substance misuse, and outpatient psychiatric clinics or community-based services. There are 351 ± 10% forensic psychiatric beds, and the length of stay ranges from 6 months to 18 years or more (the mean is 36 months). For forensic psychiatry and care of “long-stay” forensic patients in Croatia, it would be important to determine the characteristics and quality of life of forensic patients; encourage the development of aftercare and intersectoral cooperation; compare treatment models; harmonize standards, outcomes, and training, including internationally; and stimulate research and encourage international discussions.
