Crown Lengthening and Prosthodontic Considerations

Abstract

When practicing dentistry to an optimal level, the clinician is often faced with a dilemma of insufficient clinical crown or dimension to the periodontal attachment apparatus. The surgical intervention of “crown lengthening” positions the new dentogingival junction at a more apical level on the root surface so that a newly reformed junctional epithelium and connective tissue attachment can be accommodated. Reasons may vary from functional requirements, esthetic demands, or a combination of both. The clinicians thought process must hold in account what he/she must address clinically in conjunction with the patients thoughts and concerns. Is the tooth salvageable, is the biologic cost worth the price sacrificing the tissues, or what would the damage be to the supporting periodontium? We have purported to review this discussion with insight into the biological structures, techniques, and methodologies of surgical interventions and cognitive rationale for either functional or esthetic crown lengthening procedures. We review the procedures which have been developed, tested, and taught over the past many decades so that the clinician can predictably restore the dentition when it has been damaged by caries, trauma, or needs to be altered for esthetic concerns.
