Abstract

This chapter explores considerations and challenges a clinician faces when approaching disease and disability prevention for older adults. We discuss the importance of levels of prevention in this population, how statistical evidence is used to guide decision-making, and the importance of addressing health within the context of social determinants of health and cognitive, physical, and mental health. Health promotion necessitates understanding a patient’s priorities and an interdisciplinary approach. Clinicians should seize the opportunity as advocates and educators to shape expectations of healthcare providers, the community, and older adults. Finally, we highlight the importance of shared decision-making to help older adults realize their goals and optimal health.
