Abstract

Surgery plays a central role in the multimodality management of gastric cancer including staging evaluation, curative treatment, and palliation. In the staging process, laparoscopy allows for accurate assessment of peritoneal spreading. During the same procedure, peritoneal washing cytology is performed. In patients with positive cytology, hyperthermic intraperitoneal chemotherapy (HIPEC) is being explored in selected cases. Radical surgery of gastric cancer includes adequate resection margins and lymph node dissection. In patients with resectable tumors, the extent of stomach resection and lymph node dissection are the two main issues to be considered when planning radical surgery for gastric cancer. In cases with initially unresectable but nonmetastatic gastric cancer, conversion preoperative multimodality treatment is under investigation. Metastasectomy for liver or lung gastric cancer metastasis is very rarely indicated. In patients with locally advanced or metastatic disease, palliative gastrectomy can provide symptomatic relief.
