Clinical Care and Rehabilitation in Head and Neck Cancer

Abstract

Head and neck cancer (HNC) is a devastating disease affecting men and women of all ages worldwide. The complex nature of HNC presents significant treatment challenges to preserve function, minimize detrimental treatment effects, and maximize functional recovery. Achieving cancer-free survival with optimal quality of life (QOL) is the primary goal in the management of HNC patients. An integrated multidisciplinary team (MDT) of oncology physicians, speech pathologists, nurses, dietitians, social workers, dentists, physical/occupational therapists, and lymphedema therapists is needed to optimize oncologic and survival benefits. Despite the best efforts, many patients undergoing HNC management experience significant early and long-term functional changes – most significantly, but not limited to, dysphagia, dygeusia, and hyposalivation – that have the potential to negatively affect health-related QOL. To improve functional and QOL outcomes for patients with HNC, a greater understanding of the impact of early and long-term cancer and treatment-related toxicities is essential and highlights the need for evidence-based protocols that integrate intensive rehabilitation and care through a comprehensive and coordinated multidisciplinary clinical pathway. The purpose of the following chapter is to explore head and neck clinical pathways, as well as, benefits, barriers, and challenges in the management of HNC patients.
