Surgery for Malignant Parotid Gland Tumours

Abstract

Malignancy involving the parotid can arise primarily from the parotid epithelium (salivary gland carcinoma) primarily in the intraparotid lymph nodes (lymphoma) or can be due to a non-parotid cancer that metastasizes to or directly invades the parotid. The surgeon has to integrate many patient and tumour characteristics into a comprehensive surgical plan that entails a maximally accurate estimate of the required extent of surgery for the parotid gland and the regional lymph nodes. The expected extent of locoregional surgery and especially the facial nerve integrity will have to match an immediate reconstructive plan, in order to facilitate a timely and smooth administration of postoperative radiotherapy that is needed in most of the patients. The aim is radical free margin surgery whenever possible, but in many patients functional considerations will result in a situation where microscopic tumour is left behind with a realistic chance that radiotherapy can deal with this. Radiotherapy however does not compensate for inadequate surgery that leaves macroscopic disease behind.
