Sentinel lymph node biopsy analysis: towards new approaches?

Résumé

Environ 20 000 patientes par an sont susceptibles de bénéficier d’une biopsie du ganglion sentinelle (GS) en France (note de cadrage de la Haute Autorité de Santé de septembre 2011). Si l’immunohistochimie (IHC) est le gold standard de l’analyse histopathologique conventionnelle du ganglion sentinelle, il n’existe à ce jour pas de consensus sur la méthode d’évaluation du GS en peropératoire. On constate une grande disparité des pratiques entre les différents centres, le panel de technique comprend : l’analyse cytologique, la coupe congelée, l’analyse macroscopique, et plus récemment l’analyse en biologie moléculaire (OSNA®). à l’extrême, certains centres ont fait le choix de ne pas pratiquer d’analyse peropératoire au profit de l’analyse définitive plus fiable.
