Résumé
L’introduction par Charles Neer de l’hémiarthroplastie dans le traitement des fractures complexes de l’extrémité proximale de l’humérus a incontestablement représenté un tournant important dans la chirurgie de l’épaule. Certaines publications ont cependant signalé la médiocrité des résultats fonctionnels de ces arthroplasties chez des patients très âgés [1]. Ce chapitre veut tenter de proposer quelques pistes de réflexion, aptes à guider la discussion quand se pose la question d’une arthroplastie chez une personne fragile, c’est-à-dire sur un terrain où le risque d’entrée en dépendance est fonction de bien d’autres paramètres que celui de la simple fonction de l’épaule.
