Résumé
La constipation et les difficultés d’évacuation sont des symptômes fréquemment rapportés dans la population française. On estime qu’environ un quart des malades constipés chroniques se plaint principalement de difficultés d’évacuation. Ces troubles de l’évacuation se caractérisent sur le plan symptomatique par une exonération difficile, des efforts de poussées importants, des manœuvres périanales ou endo-anales défécatoires et la perception d’une évacuation incomplète. Le diagnostic positif de la constipation repose sur les critères dits de Rome par la recherche d’efforts de poussée, de selles étroites, laminées ou dures, d’une sensation d’évacuation incomplète, d’une sensation de blocage anal, de manœuvres digitales d’évacuation ou de défécations peu fréquentes (moins de trois défécations par semaine). Les anomalies de la fonction ano-rectale sont fréquentes, qu’il s’agisse de difficultés d’évacuation ano-rectale (on parle de constipation d’évacuation) ou de difficultés de retenue du contenu intestinal (on parle d’incontinence fécale).
