Place du traitement chirurgical dans la rupture aiguë du tendon calcanéen

Résumé

L’incidence de la rupture du tendon calcanéen est en augmentation notable depuis le développement considérable des activités sportives de loisir et depuis l’amélioration des techniques d’imagerie. Elle serait de 18 à 37 cas pour 100 000 personnes par an en Fonction des enquêtes épidémiologiques, avec une prédominance masculine entre 30 et 50 ans [1]. Ils’agit donc d’un motif fréquent de consultation dans les services d’urgence ou dans les cabinets spécialisés, notamment de médecine sportive, qui nécessite une prise en charge adaptée pour ne pas laisser s’installer les séquelles. Si la physiopathologie n’est probablement pas univoque pour expliquer des ruptures survenant pour un traumatisme minime ou à un âge inhabituel, l’anatomopathologie et surtout la topographie de la lésion tendineuse peuvent être également variables pour des raisons anatomiques, ce qui conduit à distinguer trois niveaux de rupture:

  • les ruptures en zone moyenne hypovascularisée, les plus fréquentes;

  • les ruptures hautes de la jonction musculo-tendineuse;

  • les ruptures basses avec désinsertion tendineuse du calcanéum.
