Résumé
Le terme générique de pesticide inclut tous les produits chimiques employés contre les parasites animaux et végétaux sur les cultures. Ces produits appartiennent à la grande famille des phytosanitaires qui comporte, entre autres, les insecticides, les herbicides, les fongicides et les rodenticides. Parmi les insecticides, les organophosphorés (OP) et les carbamates inhibiteurs des cholinestérases sont des produits potentiellement létaux en cas d’ingestion aiguë. Les intoxications souvent volontaires sont fréquentes, particulièrement dans les pays en voie de développement avec une fréquence pour les OP avoisinant 3 millions d’intoxications/ an dans le monde entier et une mortalité généralement par insuffisance respiratoire aiguë (IRA) de l’ordre de 15 à 25 % [1]. La prise en charge, essentiellement symptomatique et antidotique, doit être médicalisée dès la phase préhospitalière et nécessite le recours aux moyens actuels de réanimation. Il existe peu d’études amenant des preuves indiscutables concernant les différentes thérapeutiques utilisées; en particulier, la place de la décontamination digestive et le rôle des oximes qui demeure un sujet de controverse, notamment dans les intoxications par les carbamates. D'autres thérapeutiques sont actuellement en cours d’évaluation telles que le sulfate de magnésium ou l’alcalinisation et pourront éventuellement enrichir dans le futur l’arsenal thérapeutique dans ce type d’intoxication [1, 2].
