Résumé
La prescription d’un test diagnostique aux urgences est un art plus difficile qu’il n’y paraît. Il ne suffit pas de formuler une vague hypothèse pour qu’un test, quelle que soit sa performance dans la situation donnée, nous aide efficacement à prendre une décision. Il faut s’appuyer sur un processus cognitif solide qui se construit en quatre étapes :

évaluation la plus précise possible de la situation clinique (probabilité pré-test) ;

choix du test en fonction de ses caractvristiques intrinsèques (rapports de vraisemblance), et réalisation la plus parfaite possible ;

combinaison de l’évaluation (ou probabilité) pré-test avec le résultat du test (négatif, positif ou incertain) pour obtenir une évaluation (ou probabilité) post-test (de très faible à très forte) ;
-
puis prise de décision rationnelle (abstention, indécision ou intervention) conforme aux exigences du malade, du médecin et de la société [1].
