Abstract

The discussion on acute respiratory failure and mechanical ventilation in the elderly is very pertinent because advanced age is associated with some specific issues, including: increase of the number of elderly patients, different respiratory physiology, and increased risk of acute respiratory failure with worse prognosis; response to therapies shown to be effective in other age groups is unknown; and mechanical ventilation is responsible for a large percentage of ICU cost [1]. In addition, patients older than 65 account for more than half of ICU-patient days [2]. Finally, experimental studies suggest that aging is also associated with an increased susceptibility to injurious mechanical ventilation [3] (table I).
