Différentes études ont indiqué que l’usage de la tétine diminue le risque de mort inattendue du nourrisson (MIN). Par ailleurs, quelques études observationnelles ont suggéré que l’association tétine et allaitement aurait des effets négatifs sur l’allaitement maternel. Cependant, il n’est pas possible de s’appuyer sur des études observationnelles pour déterminer si la tétine est réellement une cause d’interruption de l’allaitement maternel. Une preuve ne peut être apportée que par des études randomisées contrôlées. Trois études de ce type ont été menées sur la relation entre la tétine et l’allaitement; mais elles sont d’amplitude réduite et ont donc leurs limites. Nous avons, dans une importante étude randomisée contrôlée, montré que, pour les mères qui ont pratiqué un allaitement jusqu’à l’âge de deux semaines, la recommandation de la tétine ne modifie en rien la fréquence de l’allaitement maternel, exclusif ou non, ni la durée de celui-ci.

Il apparaît par conséquent important que les spécialistes de l’allaitement et les agences internationales réexaminent leur position inflexible en décourageant tout usage de la tétine. Il apparaît logique qu’une fois que la mère a suffisamment de lait, l’usage de la tétine n’influence en rien la durée de l’allaitement lorsque celle-ci souhaite allaiter plus de trois mois, alors qu’il y a en plus la possibilité de diminuer le risque de MIN.
