Quel équilibre thérapeutique en cas de micrométastases ganglionnaires?

What therapeutic balance for node micrometastasis?

Résumé

Les micrométastases ganglionnaires correspondent à des atteintes > 0,2 et ≤ 2 mm et les sub-micrométastases à des cellules isolées ou amas cellulaires ≤ 0,2 mm. Leur fréquence (40 à 45 % des ganglions sentinelles GS envahis) [1] a augmenté du fait de leur recherche sur quelques GS, avec des méthodes d’analyse adaptées à la découverte d’atteintes de petite taille (coupes sériées et immunohistochimie ou IHC). La reproductibilité de distinction entre micro- ou sub-micrométastase est médiocre, même auprès de pathologistes spécialisés. Les métastases dites «occultes » peuvent correspondre essentiellement à ces catégories d’atteinte, mises en évidence sur des coupes sériées et analyses avec IHC [2].
