Résumé
Le sepsis est le principal domaine de recherche pharmaceutique en réanimation. La prise en charge des patients septiques est basée sur différentes stratégies parallèles. Tout d’abord, le foyer infectieux doit être contrôlé par la chirurgie, le drainage percutané et une administration précoce d’antibiotiques [1]. La place des nouveaux agents anti-bactériens sera donc discutée ci-dessous. Ensuite, en complément du traitement symptomatique, la modulation de la réponse de l’hôte est une piste thérapeutique poursuivie depuis longtemps. Dans cette optique, seule la protéine C activée, avec ses propriétés anti-coagulante et anti-inflammatoire, a démontré son efficacité dans le sepsis grave. Pourtant, la confirmation de l’efficacité de la protéine C activée semble difficile à obtenir. Ainsi, à ce jour, le seul traitement ayant prouvé une amélioration de la survie reste l’antibiothérapie (associée aux mesures symptomatiques de réanimation).
