Résumé
La réanimation initiale du choc septique implique de remplir un certain nombre d’objectifs thérapeutiques dans un délai court qui va de soixante minutes pour les mesures de sauvegarde initiale jusqu’à six heures maximum pour les mesures complémentaires. Les objectifs thérapeutiques ont fait l’objet d’une analyse exhaustive et argumentée dans un texte de 2008 publié sous l’égide d’un groupe d’experts coordonné par R.-P. Dellinger (Surviving Sepsis Campaign: International guidelines for management of severe sepsis and septic shock. Crit Care Med 36, pp 296–327). (Le système GRADE a été utilisé pour préciser la force des recommandations faites par les experts [GRADE working group BMJ 2004, 328, pp 1490–1498]. Ceux-ci (55 experts internationaux) ont utilisé la méthode Delphi pour l’établissement des recommandations avec des réunions physiques, des téléconférences et des discussions par courriers électroniques. Les recommandations de niveau 1A doivent impérativement être suivies, les recommandations de niveau 2D font l’objet de débat et leur pertinence n’est pas établie).
