Résumé
Les opioïdes sont depuis longtemps connus et utilisés pour leurs propriétés antinociceptives. Durant la période péri-opératoire, la gestion de la composante nociceptive de la douleur, prédominante sur le plan physiopathologique, fait ainsi appel depuis de nombreuses années à cette classe pharmacologique. Cet usage commun des opioïdes dans la pratique anesthésique a permis d’en apprécier les avantages tout autant que les risques, avec pour corollaire la rédaction de recommandations professionnelles. D’autre part, l’avènement de nouvelles molécules et méthodes de délivrance, dans un environnement de pratiques sans cesse plus sécurisées et contrôlées, a amené les prescripteurs à réfléchir non plus uniquement aux effets bénéfiques et secondaires immédiats, classiques, mais également à ceux identifiables à distance de la période péri-opératoire, en particulier les effets retardés sur l’allostasie antalgique [1].
