Résumé
La chirurgie reste au centre de la thérapeutique de la plupart des cancers. Un cancer localisé en résection complète est susceptible de guérir. Mais le chirurgien ne sera plus jamais le seul spécialiste concerné, comme il a pu l’être dans le passé. Les développements de l’oncologie moléculaire, qui approchent désormais les tumeurs, y compris solides, sous l’angle d’attaque de l’expression de biomarqueurs, ou de mutations génomiques, ont ajouté aux missions du chirurgien un rôle essentiel dans l’approvisionnement des tumorothèques, véritables ? cambuses thérapeutiques ? de demain. La résection chirurgicale reste encore aujourd’hui le seul traitement en mesure de guérir définitivement certains cancers. Mais ce privilège répond à des exigences particulières, et la chirurgie oncologique est encadrée par des contraintes spécifiques qui dépassent largement celles de la chirurgie d’exérèse pour lésion bénigne. Les pouvoirs publics dans leur démarche d’accréditation des centres habilités à la chirurgie des cancers ne font que relayer les sociétés savantes qui, depuis une décennie, émettent des recommandations qui deviendront inéluctablement opposables à plus ou moins brève échéance. La notion de seuil d’activité en est un bon exemple. Il est en effet aujourd’hui clairement démontré que, pour les patients opérés de cancer, le pronostic est meilleur lorsqu’ils sont opérés par des équipes ayant une expertise en oncologie et une activité importante. Les développements techniques de tout ce qui entoure l’acte chirurgical (imagerie, endoscopie, biologie, anesthésie, réanimation...) ont apporté des informations et des possibilités nouvelles, qui affinent les indications, mais aussi renforcent le principe de précaution.
