Résumé

La prise en charge des hémoptysies est difficile. Ce saignement provenant des voies aériennes sous-glottiques et du poumon peut en effet se limiter à une expectoration d’abondance minime qui restera isolée ou sera le précurseur d’un saignement massif, sinon être d’emblée abondant pouvant, dans les formes extrêmes, inonder les poumons conduisant à l’asphyxie, la défaillance circulatoire et la mort. Les conséquences cliniques dépendent plus du débit de l’hémorragie que de son volume, de l’étiologie bénigne ou maligne, et de l’état pulmonaire et cardiaque du malade. La démarche diagnostique est indispensable dans tous les cas. Elle repose sur un faisceau d’arguments découlant de l’examen clinique, de la radiographie thoracique, du scanner, et de l’endoscopie. Sept à 20% des hémoptysies resteront cependant cryptogéniques.
