Résumé
L’accident vasculaire cérébral (AVC) représente un des enjeux majeurs des politiques de santé publique, avec une incidence de 125 000 cas par an, ce qui le place au troisième rang des causes de mortalité après les cardiopathies et les cancers, ainsi qu’au premier rang des causes de handicap physique d’origine non traumatique chez l’adulte. La reconnaissance de cette pathologie complexe s’illustre notamment par la structuration de sa prise en charge à travers la mise en place de filières de soins dédiées. Les principes de gestion préhospitalière ont pu également être soulignés par l’édition de recommandations de bonne pratique par la Haute Autorité de santé (HAS) en mai 2009 concernant l’alerte, la phase préhospitalière, la phase hospitalière initiale et les indications de la thrombolyse [1]. Si le développement quasi simultané de la thrombolyse et de l’intérêt des unités neurovasculaires (UNV) a démontré que certains types d’AVC pouvaient bénéficier de thérapeutiques curatives, cette avancée a également souligné l’importance primordiale de la régulation médicale de ce type de pathologie en préhospitalier, tant par le respect des délais que par l’orientation optimale.
