Intervention de Tyrone David dans les dissections aortiques de type A

Résumé

La popularité grandissante des interventions de conservation de la valve aortique par réimplantation (intervention de Tyrone David) dans le traitement des a névrysmes de la racine aortique et leurs bons résultats à moyen terme (1, 2) ont conduit certaines équipes à la proposer dans le traitement des dissections aortiques de type A (36). Si un certain consensus technique règne pour le traitement chirurgical des dissections aortiques de type A (7, 8), malgré l’absence de données répondant formellement aux exigences de la médecine «fondée sur la preuve», l’indication de l’intervention de Tyrone David dans les dissections aortiques de type A reste en revanche très débattue. La controverse est nourrie par certaines des caractéristiques de cette intervention pouvant être perçues comme incompatibles avec l’objectif essentiel du traitement chirurgical des dissections aortiques de type A: sauver la vie du malade (9).
