Abstrait
Définis par un volume tumoral important, les cancers du sein localement évolués et les récidives locales agressives associent un groupe hétérogène de malades comportant des pronostics très variables. Du point de vue de la technique chirurgicale, leur dénominateur commun est la nécessité de réaliser des résections locales étendues nécessitant une couverture pariétale avec du matériel autologue, le plus souvent des lambeaux musculo-cutanés. Qualifiée de maladie inopérable par les techniques classiques de mastectomie, la littérature confirme que la primeur thérapeutique est donnée aux traitements systémiques pour combattre la maladie micrométastatique (1). Dans ces conditions, quelle peut être la place d’un traitement local chirurgical agressif?
