Résumé
Dans les sociétés occidentales, l’incidence de la lombalgie commune varie de 60 à 90 % et sa prévalence est de 5 %. Les prises en charge sont multiples mais principalement axées sur le concept biopsychosocial. Ce concept est certes utile pour le thérapeute et pour le patient mais ne doit pas nous affranchir d’une démarche active de recherche lésionnelle. Certains travaux récents issus de la biologie et de l’imagerie ont permis d’avancer dans le domaine lésionnel et plus particulièrement sur la compréhension de la dégénérescence discale. Ils ont montré le rôle majeur de l’apoptose des cellules discales, de l’inflammation du disque et du stress mécanique dans la genèse et l’entretien de la dégénérescence discale. Ces phénomènes cellulaires et moléculaires pourraient être bénéfiques ou délétères pour le patient suivant leur niveau de régulation. Ils pourraient à terme aboutir à de nouveaux traitements spécifiques intradiscaux. La démarche lésionnelle doit plus que jamais être systématiquement entreprise dans la lombalgie commune en 2007.
